Accident Report

An individual can complete their own collision report if an officer does not investigate the collision at the scene. Please click on the title to retrieve the vehicle collision report form. Once completed, this form is mailed to Washington State Patrol (WSP). You may also purchase a copy directly from the WSP site.

Illegal Drug Activity Tip

Concealed Pistol License Application

This application is for citizens who live within the city limits of Oak Harbor to apply for a State of Washington Concealed Pistol License.  Applications are accepted at the OHPD on Tuesdays and Thursdays between the hours of 10:00 a.m. - 1:30 p.m.

WATCH:

The Washington State Patrol Identification and Criminal History Section (WASIS) has established a web site as the official Internet source providing criminal history conviction records for the state of Washington only.

Vacation Watch:

This form is for citizens who live within the city limits of Oak Harbor to request a residential vacation watch (30-day maximum) of your home.
 
Firearms Dealer License:

This application is for firearm dealers located in the City of Oak Harbor to apply for an annual State of Washington Dealer License. Applications are accepted at the OHPD on Tuesdays and Thursdays between the hours of 10:00 a.m. - 1:30 p.m.

Junk Vehicle Verification

This form is to request a Junk Vehicle sign-off for a vehicle located in the City limits of Oak Harbor. Once completed, citizens should contact ICOM Dispatch at (360) 679-9567 to request a law enforcement officer to inspect and authorize the disposal of the junk vehicle.
